Looks pretty fake to me. Those veins look photoshopped. Then again, with something that freakish looking, there's no way of telling quite how 'real' something normal might look.

Please don't tell me that you know someone that has info that it's real. I've actually come across this pic+argument before, and it's like one of those wacky internet myth things or something. Back on the subject of massive nuts... I'm pretty sure everyone has seen that video of the man who sits on his own testicles. I remember seeing it, thinking he was sitting on a pillow, then he stands up and the pillow was hanging between his legs. I still have nightmares.

I think it is a bit of a misnomer to call them "nuts" at that stage. Maybe "worm sack", or if you prefer, "parasite bag" would be a better discription
