created Cable Management for Neat TV/AV Installation?

Hey guys,
Slightly O/T here but I know there's probably a lot of you here who've faced this same problem and likely come up with ingenious solutions.
I've just bought my first house, and inevitably (being male) the first thing I'm thinking about is how to hook up my home theatre system.
Since the house is bought and not rented, we're free to get busy with the power tools, although the house is actually a new build so I don't want to screw things up too much.
I want to have my TV mounted on the wall, but I have no idea of the best way to hide the cables. I don't have access to the rear of either of the walls I'm planning to mount a TV on, one is timber stud with a plasterboard skim, the other is a blockwork external wall again with a plasterboard skim.
I'm assuming I will need a power cable, and then multiple HDMI cables to the various sources, and I'm not sure where to mount them either yet. Ideally I'd like to mount the speakers up on the wall too, which will obviously have cables going to them as well.
Any tips/advice/suggestions on how to do this neatly and relatively easily? I'm happy to DIY but I'm certainly no builder.
